Jay Eastridge, Pharm.D.
Jay Eastridge graduated with a Doctor of Pharmacy degree from Mercer University Southern School of Pharmacy in 1993. Over the past 31 years, Jay has gained experience as a staff and clinical pharmacist at Taylor Regional Hospital. He later served as Director of Pharmacy and Clinical Services Administrator at Taylor Regional Hospital. Jay gained home infusion experience as Director of Pharmacy for CertaCARE. He has been married to Jessica Davenport Eastridge for the past 33 years. Jay and Jessica have three children; Lauren, John and Chloe. They also have a son-in-law, Mike, a daughter-in-law, Alyssa and a son-in-law, Chanson. Jay and Jessica have six grandchildren, Easton, Hudson, Jaxson, Jett, Jrue, & Audrey. Jay is an active member of 3Trees Church. He enjoys spending time with family, listening to music, yard work, do-it-yourself projects, and playing golf.

Chad Phelps, Pharm.D.
Chad Phelps graduated with a Doctor of Pharmacy degree from the University of Kentucky in 1999. After graduating from UK, Chad returned to his hometown of Greensburg with his wife, Janet Phelps, also of Green County. They have been married for 26 years, and have two children, Reed and Reagan. Chad has worked as a retail pharmacist in Campbellsville and Greensburg for over 25 years. He has years of experience as a pharmacy manager, working to educate his patients while advocating for them so that they may receive the best care possible. Chad enjoys spending time with his family and close friends. His hobbies include motorcycling, scuba diving, hiking, canoeing, snorkeling, collecting knives, and attending live music events.

Brooke Sprowles, Pharm.D.
Brooke Sprowles graduated in 2006 with a Doctor of Pharmacy from the University of Kentucky College of Pharmacy. Originally from Middleburg in Casey county, after graduation she moved to Campbellsville with her husband, Eric Sprowles. Brooke has worked as a retail pharmacist since graduation as both Pharmacy Manager and a staff pharmacist. Brooke and Eric have 2 children, Riley and Holden. Her hobbies include reading, baking, going to Cincinnati Bengals football games and watching her children play sports.

Danielle Smith, Pharm.D.
Danielle Smith graduated with a Doctor of Pharmacy degree from the University of Kentucky College of Pharmacy in 2011. After graduating, Danielle returned to her hometown of Campbellsville with her husband, Adam Smith, also of Campbellsville. Danielle and Adam have 2 children, Addison and Dawson. Danielle joined the Eastridge-Phelps Pharmacy family in 2019 but has worked in retail pharmacy since 2005. Danielle is an active member at Pleasant Hill Baptist Church and enjoys playing piano and singing in the worship team. She also enjoys spending time with family, baking, watching the Green Bay Packers and the Kentucky Wildcats.
